You have your passport in your possession to submit with your application.

Your passport is undamaged other than normal wear and tear.

Your passport was issued when you were age 16 or older.

Your passport was issued within the last 15 years.

Your passport was issued in your current name or you can document your name change.

Check the I have read the privacy box then click submit

The left option is Fill out online and Print  click Submit

Fill in your name  date of birth,  city of birth,  state of birth,  country of birth, and ss number

then Gender, height, hair color, eye color, occupation and employer

then method of communication

fill in email and phone number

When is your next trip (make it 60 days in the future ) and return  and where you expect to go  Any country will do

Enter emergency contact & info

NEXT

select the passport you now have and questions about it

Answer questions about your passport

list any other names you have used

NEXT

REVIEW YOUR ANSWERS

Check you want passport book and choose the processing method
